Disclosure Troubleshooting Guidelines

✅ Can’t find disclosure?

  • The Disclosure form is only visible when logged in under an ACGME role that requires Disclosure access, such as: 
    • Board Member
    • Review Committee Member or Ex-officio
    • Recognition Committee Member or Ex-officio
  • For example: If you log in as Program Director, you will not see the Disclosure link because that role does not have Disclosure permissions

✅ How to resolve the issue

  1. Toggle to the correct role:
    • Click the down arrow next to the person icon in ADS.
    • Select the profile for ACGME 

  • Once switched, the Disclosure option will appear in the dropdown menu.

  • If you have multiple roles (e.g., Program Director + RC Member):
    • Ensure you are logged into the correct role before attempting to access Disclosure.
    • Your token and permissions are rebuilt when you switch organizations/roles, so toggling is essential. 
  • If you still get a permission error after toggling:
